Report/Complaint to the Police


Was a report made to the police by:
(a) a victim of the alleged offence?
(b) a witness of the alleged offence?

What is this person's name?
What is the date of the report?
What is the name and official title of the investigator to whom the report was made?
What was the nature of the criminal offence that was reported and the injury suffered?

Detention

If you were detained for more than 24 hours, for each extension of your detention (which by law must not exceed 400 days in total):
When did the extension begin and when did each extension end? - upload each detention extension order.
How did the authorities justify this?
Were you detained in:
(1) a prison?
(2) placed under House Arrest?
(3) placed under Zone Arrest?
Were you mistreated? - if so, upload photographs and/or medical reports.
If you were released what were the conditions of your release?

Prosecution (Penuntutan)

Was a Letter of Indictment (Surat Dakwaan) issued? - if so upload it.
Did the Public Prosecutor at any stage issue a Termination of Investigation (Surat Perintah Penghentian Penyidikan or SP3)? - if so upload it
What was the date of the SP3 and the date and time of your release?

Trial

Did you receive a summons to trial? - if so upload it.
Was your identity verified in court and possible conflicts of interest examined?
Were you asked whether or not you understood the Letter of Indictment (Dakwaan)?
Was the Letter of Indictment challenged – if so on what grounds?
Name any factual witnesses that appeared and who cross-examined them.
Name any expert witnesses that appeared and who cross-examined them.
Were you required to give evidence?
Was all the written and forensic evidence used shown to you (for example, indicate any reports and/or crime scene evidence that were not shown to you)?
What was your defence (Pleidooi/Pembelaan)? - upload it.
What was the Prosecutor's response to your defence (Replik)? - upload it.
Did you submit a further defence (Duplik) in response to the Prosecutor's response? - if so, upload it.

Judgment

Which judgment was meted out: - upload the written judgment of the judges.
1.Acquittal – not guilty?
2.Dismissal – no valid criminal offence?
